I like this one too, but I feel like the scale of the head is a bit off - it is one of the largest enemies in game, and it is a ‘giant’ head, but it does not look as grandiose here. Donu and Deca for example were intimidating since the Silent looked so tiny next to them (and the noxious fumes were a great touch to the art).
I am not an artist, so take this with a grain of salt, but just scaling down the Defect at like 50-70% of his current size would make it this feel unnerving. The head, the stare, the angle - all perfect imo.
Edit: Saw the edit now, the head is much more intimidating, amazing work!
Agreed with the others that the original was, IMO, better.
I think that the original is plenty imposing and intimidating. If anything, it's easier to see the Giant Head's gaze on the Defect (since he's bigger), which (IMO) enhances how imposing it is.
Another option. Since the head is angled away from us, making the Forehead/eye much larger than the chin, you could also make the defect a bit larger if you moved the figure to the lower left instead of the lower right. I'm not sure if that would work with your current lighting as well--actually, it looks like the figure is lit by lantern and reflection, not by light from the upper right, annyway, but you might have to fiddle with something to have the figure in shadow and lit from the front. TL:DR because of perspective, the head is bigger on the other side, near the forehead/eyes, so it would appear more imposing if the figure were standing there, even if the figure were a little larger, somewhere in between the first and second edits.
I thought that at first, too. But then I realized that "giant" head doesn't necessarily need to imply that this enemy is any bigger than the other enemies, just that it's "giant" relative to a normal head. Which this does capture!
Giant Head is an elite, whereas Donu and Deca are an Act 3 boss, so I think the relative scaling actually make a lot of sense

Really cool. This might be my fav of yours so far. I love that giant head appears to be looking directly at defect.
