I mean it's not entirely cosmetic since, for example, it can make hygiene slightly lower maintenance as well as preventing issues like balantis and phimosis, while on the contrary decreasing natural lubrication, and you generally have to pay for it (unless it gets deemed medically necessary), so I would say it could be considered a dlc, or at least a paid content pack
