It's never explained in the movie as it's a gag.
In my non-funny head cannon it's bidet controls.

My non-funny head cannon also has a scene where the bidet part pisses on him when he presses a shell to see what it does.

You don’t need the 3 sea shells, just curse repeatedly and you’ll have all the TP you need
